Observation

110-300-0165(2)(c) Licensee had cords accessible to the children that could create a loop around a child’s neck in five-year-old room from a phone charger and one black and white cord not attached to the wall by the teacher’s desk area. Licensee had a ten foot rope laying outside the door of the four year old classroom accessbile to all children. 110-300-0165(2)(d) Licensee had plastic bags accessible by the hand washing sink in the three-year-old classroom, plastic bags accessible in the closet area in the five-year-old room and there were plastic bags on the shelve upstairs leading to the exit area. 110-300-0165(3)(a) Licensee had a razor cutting device accessible by the upstairs exit door on a shelve from a press and seal box. 110-300-0165(3)(f) Licensee did not have the following items anchored to the wall creating a tipping or falling hazard, in the 12-24-month room there was a garbage can on a shelf, and a black shelf that was not attached to the wall. In the 3-year-old room- year old room, there is a grey shelf not attached to the wall, cubbies, blue clear shelves, book shelve, and a blue shelf by the sink that are not attached to the wall. In the four-year-old room, there are white clear bins not attached to the wall. In the five-year-old room, there is a blue shelf not attached to the wall. Upstirs in the exit pathway, there are several items on a shelf that due cause a tipping hazard and one large white shelf that Is not anchored to the wall. 110-300-0165(3)(g) Licensee has rusty staples on the outside chalk board that are accessible to the children.
